Can you move the attractor further back into the cage? It looks like the cat would have to squeeze under the attractor to place much weight on the pan?
Also the chain hanging down is going to guide the cats head, which guides his feet as the picture shows.
It looks like the cat went in as far a he comfortably could.
Some minor modifications would stop those misses!

Looks like you ought to night latch your trigger. I bet somthing moved or sqeeked before he committed to that last step .I have seen that on my trap pans before clearly defind prints allaround the pan and the one on the pan is indistinct . I have always found that the trap pan has some false drop in it .when i see this

Looks to me like you need to guide his step a little better. You may be at four pounds on the leading edge of your treadle but where that cat stepped (close to the hinge point) will be greater than that. Step stick two inches in front of the leading edge and another just to the front of the hinge point will land his step right where you need it. Improve your attractor and add some appeal to the bottom rear of the cage and the cat will be committed to that fateful step. If he bends down to smell something he will have all his weight on his front foot. In your picture it looks like all his weight was on his left foot and his right merely kept his balance while checking out your attractor that was overhead. That cat should have got caught. With a few adjustments I think you could get him. Good luck.
